Description

Provide leadership and supervision to a team of nurses and home health aides.Develop and implement home care services demo, care plans, ensuring that they are individualized and based on patient needs and preferences.Conduct comprehensive assessments of patients' health status, including physical, psychological, and social factors.Coordinate and schedule home visits for patients, ensuring adequate coverage and timely delivery of care.Collaborate with physicians, therapists, and other healthcare professionals to develop and modify patient care plans as necessary.Monitor and evaluate the quality and effectiveness of care provided by the team, identifying areas for improvement and implementing corrective actions.Provide clinical guidance and support to the nursing staff, assisting with complex patient cases and addressing any concerns or issues.Maintain accurate and up-to-date documentation of patient care activities, including assessments, interventions, and outcomes. Excellent communication and interpersonal skills, with the ability to interact with patients, families, and healthcare professionals.Strong abilities to demonstrate company services to clientsExperience in IV therapy, ICU, pregnancy & Post-Partum care, Elderly careMinimum of 2 years of nursing experience, preferably in home health care or a related field.Strong clinical assessment and critical thinking skills for lead conversionIn-depth knowledge of home health care regulations, policies, and best practices.Proficiency in using electronic medical record (EMR) systems and other relevant software.Ability to prioritize tasks, manage time effectively, and work independently.Flexibility to adapt to changing needs and situations.
