We are thrilled to present an exceptional opportunity for HMR-accredited pharmacists across Australia. Your pivotal role involves collaborating closely with our dedicated Chemist2U HMR support team and community doctors, where you will be responsible for conducting HMRs in the community and producing comprehensive reports. This engaging and rewarding position offers a chance to contribute significantly to the enhancement of patient care within the community.

Key Responsibilities:

As a pharmacist, you will:
• Book HMR appointments that mutually suit you and the patient using the nominated referral platform
• Complete prescreening questions within the platform
• Conduct comprehensive Home Medicines Reviews (HMRs) to assess whether patients’ medications are being managed safely, effectively, and optimally to facilitate the best health outcomes
• Write comprehensive medication reports that will be sent to the referring general practitioners
• Process the finalised HMR consultation and submit it to Medicare to facilitate the reimbursement process.

Key Requirements:

To be a successful applicant, you will need the following qualifications and attributes:

· Have full working rights in Australia.

· Possess complete AHPRA registration and maintain professional liability/public indemnity insurance coverage.

· Hold AACP Accreditation/HMR accreditation.

· Exhibit exceptional written and verbal communication skills.

· Demonstrate adeptness in working independently as a sole practitioner while fostering strong relationships with our Chemist2U Support Team.

· Uphold a high standard of ethical practice as a contractor and a Chemist2U representative.

· Display unwavering commitment to providing exemplary customer service to patients and their referring general practitioners.

· Provide empathic care to patients in the community.

· Previous experience conducting HMRs in the community.

Desirable Criteria:

At least one year’s experience conducting HMRs in the community.
